    Planning a Safe Return to Hockey
    Following the course, the participant Hockey Canada Registry (HCR) profile will be updated. If you do not have an existing profile in HCR, we request that you print/save your certificate of completion as a PDF or take a screenshot. You must complete the five (5) question survey at the end of the course in order to be issued your certification for the course.

    2020-2021 Team Official Qualification & Requirements
    One key element of the requirements is the mandatory Hockey University - Planning a Safe Return to Hockey course that is required for all team officials. Coaches in divisions U9 (formerly Novice) and below must hold Coach 1 - Introduction to Coach. No other qualifications are accepted. Coaches in divisions U10 (formerly Minor Atom) and above must hold Coach 2 - coach Level "Trained" or higher.

    City of Markham COVID-19 Acknowledgement, Waiver, and Indemnification
    Everyone (players and bench staff) must complete this waiver prior to the first ice session.

    Rowan's Law Acknowledgement
    All volunteers (coaches, trainers, managers, and on-ice volunteers) must review this information and acknowledge that they have done so in order to be rostered with the Markham Waxers for the hockey season.

    Respect in Sport
    Bench staff must take Respect in Sport for Activity Leaders. One parent of every player in the OMHA must take Respect in Sport for Parents before their child can be rostered to play.

    Gender Identity & Expression
    Training module for team officials, to support trans-inclusive hockey in Ontario, Understanding Discrimination based on Gender Identity and Gender Expression Training. There are two modules which will educate team officials on this important topic. All team officials will require an eHockey account in the Hockey Canada Registry (HCR) in order to register for and complete the two modules. Once completed, notification will be posted to the team officials’ member profile on the HCR.

    PVSC Volunteer Letter Request
    Request a letter that identifies you as a volunteer of the Markham Waxers. This letter is required when you apply for a Police Vulnerable Sector Check (PVSC).

    York Regional Police Online PVSC Application
    Note: you must apply for a Police Vulnerable Sector Check in the region in which you reside. If you reside in Toronto, you must contact the Waxers Administrator for an application form.

    Application for Conversion/Upgrade of Certification
    To convert certifications taken outside of the OMHA, bench staff must complete the conversion application and submit the form and fee at the OMHA office.
