Complete this registration form to attend evaluations for rep and select hockey with the Markham Waxers. You need only complete this form once (before the first tryout) for each category (AAA, AA, A, BB, or select) for which you are trying out. When you arrive at the evaluation, check in at the registration desk. There you will pay for the evaluation and receive a jersey to use during the evaluation. Jerseys are to be returned to the coach at the end of each evaluation. Payment of the evaluation fee ($26) is by credit or debit card or Apple Pay only. No cash.

Tryout Paperwork

Even if you upload a PDF of your evaluation paperwork, bring it with you and present it at the registration desk prior to the first evaluation for each team. Any player not presenting the proper paperwork will not be allowed to participate in the evaluation. Players who reside in Stouffville and Richmond Hill and played A or AA last season, must get a PTS from their home MHA to attend Waxers AAA evaluations. U14 through U18 players who played on a non-OMHA team during the 2022-2023 season must submit a hard copy PTS to try out with the Waxers. Players will not be allowed on the ice without the proper paperwork.
