2024 Evaluations Registration, Waxers A (Markham Waxers)

Complete this registration form to attend evaluations for rep and select hockey with the Markham Waxers. You need only complete this form once (before the first evaluation) for each category (AAA, AA, A, BB, or select) for which you are trying out. When you arrive at the evaluation, check in at the registration desk. There you will pay for the evaluation and receive a jersey to use during the evaluation. Jerseys are to be returned to the coach at the end of each evaluation. Payment of the evaluation fee ($30) is by credit or debit card or Apple Pay only. No cash.

We will be given a jersey to use during the evaluation and will return the jersey at the end of the evaluation to the coach.^*
