New Player Profile Request Form

Fill out and submit this form if you are registering a player who is new to organized hockey in Canada. Upon receipt, the Waxers will use this information to create a Hockey Canada Registry (HCR) player profile for your child. Once complete, we will email you your child's HCR number for registration. Please allow up to 7 days for this form to be processed and your child's profile to be created.
