The Lake Ontario Region (LOR) is a geographical subdivision of the Ontario Hockey Federation (OHF) that includes the Greater Toronto Hockey League (GTHL)—Toronto, Mississauga, Vaughan, Thornhill, and Scarborough—and the surrounding Ontario Minor Hockey Association (OMHA) area including Oakville, Brampton, Richmond Hill, Markham, Ajax, and Pickering.

A player in the LOR who plans to attend the evaluations of teams in the U14 to U18 (except U18 AAA) of a different LOR member from the one with which they were last registered MUST obtain a Permission to Skate (PTS) form and present it at the evaluation. Players will not be allowed on evaluation ice without a PTS if required. PTS forms MUST be acquired from the association with which the player was registered during the past season.

PTS Processing

  • PTS forms will be issued electronically only via our website. 
  • PTS forms will not be issued at the Waxers office.
  • The Waxers only issue PTS for players who played with the Waxers during last season. (exception: Stouffville and Richmond Hill residents who played on a Stouffville or Richmond Hill rep team during last season requesting a PTS to play AAA in the LOR, see procedures below) 
  • Players who participated in house league and select do not need a PTS to try out for other teams.

Please allow a minimum of 48 hours for processing of PTS forms. They will be either approved or denied prior to the date of the tryout, provided they were submitted a minimum of 48 hours prior to the tryout date.

How to Submit a PTS Request

Please follow the instructions below to submit your request.

LOR/Jr/U18 AAA PTS

  1. Go to www.minorhockeyforms.com

  2. If you have never used the system before, you will have to first create an account.   Click on Register on the top right of the screen and then complete the Sign Up form. Please ensure that your email address is correct and that you take note of your password. You will need this information in order to log in again to download your PTS once you receive notification that your request has been approved.

  3. Once you are registered and logged in to the site, select “Markham AAA Zone” from the list if you played on a Waxers AAA team during last season or select "Markham Waxers Hockey" if you played on a Waxers BB/A/AA team during last season. If you choose any other association, your request will be rejected and you will have to resubmit to the Waxers.

  4. Select the form for which you are applying. There are three options:

    • Lake Ontario Region (LOR) Permission to Skate
    • OMHA Junior Player Tryout Permission Form
    • OMHA U18 Player Tryout Permission Form (August tryouts only)

  5. Complete the questions on the form ensuring that all the information is accurate and then click Submit.

  6. Log Off.

  7. You will receive an email confirmation that your request(s) have been submitted and an email will be sent to the Waxers advising us of the request(s). Please do not email us to check if your request has been received. If you receive the confirmation immediately after submitting the form, your request has been logged.

  8. VP AAA and VP A/AA will review all PTS requests. If your request has been approved, you will be sent an email advising you. You then log back into the Minor Hockey Forms system to download and print the approved PTS form. If your request has been denied, you will receive an email with an explanation as to why it was rejected.

Stouffville and Richmond Hill Resident PTS Processing

Stouffville and Richmond Hill residents who played on a Stouffville or Richmond Hill rep team during last season who wish to try out for a Waxers AAA team in the Lake Ontario Region (LOR) must follow the procedure below.

  1. Apply for a PTS from Whitchurch Stouffville Minor Hockey Association or Richmond Hill Hockey Association.
  2. Email the PTS to VP AAA Stephen Gaunce at [email protected]
  3. Apply for a PTS from the Waxers AAA Zone

Stouffville residents who played on a Stouffville rep team during last season who wish to try out for a AAA team in a non-LOR centre must follow the procedure below.

  1. Apply for a PTS from Whitchurch Stouffville Minor Hockey Association or Richmond Hill Hockey Association.
  2. Email the PTS to VP AAA Stephen Gaunce at [email protected]
  3. Apply for a waiver from the Waxers AAA Zone

Stouffville and Richmond Hill residents who played on a Waxers U13, U14, or U15 AAA team during last season who wish to try out for a AAA team in the Lake Ontario Region (LOR) must follow the procedure below.
