City of Markham Acknowledgement, Waiver, and Indemnification
Every on-ice participant must complete this form prior to their first on-ice session.
Certificate of Insurance Request
Complete this form to request a certificate of insurance for Waxers team-related and approved activities. The process can take up to 10 business days.

Room Rental Request

Complete this form to rent a room in a City of Markham facility for a Waxers team meeting/event. Submissions must be made a minimum of one week prior to the event. The team will be invoiced for the cost of the rental.

Waxers Rep Players' Banquet Reservations & Awards Nominations

RSVP your team for the Players' Banquet and nominate your team's award winners. Form must be completed no later than March 1. We cannot guarantee full participation for teams who provide information after the deadline.

Neil Bullock Scholarship Application

The Neil Bullock Scholarship is an annual award of $500 for post-secondary education. Any player in their last year of secondary school and rostered to a Markham Waxers team is eligible. One winner will be chosen from rep applicants and one winner will be chosen from house league applicants. To apply, complete this application and upload your essay and first report card of the school year. Deadline is March 15.
