Proof of Vaccination Requirment to Play
The OMHA Vaccination Policy requires all OMHA participants born in 2009 or earlier to be fully vaccinated by October 31.

This policy applies to the following Affected Persons:
  • Players born in 2009 or earlier
  • Team Officials (Coaches, Trainers and Managers, etc.)
  • Game Officials (referees, linespersons, timekeepers, etc.)
  • Association Officials (minor hockey association directors, officers, executive and staff)
  • OMHA Officials (directors, officers, executive and staff, including hockey development personnel)
  • Volunteers
  • Instructors (third parties engaged by Associations or Teams to provide specialized instruction (e.g., power skating, goaltending coaching)
 At present, this policy does not apply to hockey players born in 2010 or later or to hockey spectators.
 
In order to continue participating, all Affected Persons (listed above) must provide either proof of vaccination or a copy of an OMHA accommodation by completing the 2021 COVID Vaccination Verification form on the Waxers website.

FAQs
My child was born after 2009. What do I need to submit?
Nothing. The policy does not apply to hockey players born in 2010 or later. We are sending this to all house league members to ensure we reach all volunteers. If your child was born after 2009 and your family has chosen not to volunteer, you can disregard this email.

As a parent who attends games, do I need to upload my proof of vaccination?
No. However, the City of Markham requires proof of vaccination to enter their facilities, so you will need to provide proof at the door along with the City of Markham's screening form.

My child was born in 2009 but has only had one dose. 
Your child is ineligible to participate until they are fully vaccinated and you have uploaded proof of full vaccination. “Fully Vaccinated” means fourteen (14) days after an individual has received a completed series of an Accepted COVID-19 Vaccine. 

Is there any financial reimbursement for lost weeks?
No.

My child was born in December 2009 and is not yet vaccinated. Can he continue to participate?
No. All players born in 2009 or earlier must submit proof of full vaccination by November 4.

Is there any leeway with the due date?
No. All proofs of vaccination must be uploaded by November 4 in order to be eligible to participate.

What is considered valid proof of vaccination?
Download your COVID-19 vaccine certification from the government of Ontario website.
 
I am a volunteer but do not go on the ice. Do I need to submit proof?
Yes. All volunteers, whether participating on the ice or on the bench, must upload proof of vaccination.

My child cannot be vaccinated for medical reasons. What do we do?
In order to continue to be eligible to participate in any Waxers activities you must provide proof of an approved OMHA Accommodation. You can apply for one online. It can take up to two weeks to process an accommodation request and you are not eligible to participate while waiting for a ruling on your accommodation request.
