Every participant (players, coaches, trainers, bench staff, on-ice volunteers) in the OMHA must provide proof of full vaccination (including 14 days post second dose) in order to participate in OMHA activities after October 31. Your proof of vaccination documentation will be deleted after it's verified. It will NOT be retained.
