Proof of Age and Residence Submission

Hockey Canada and the Ontario Minor Hockey Association (OMHA) require proof of residence and proof of age. OMHA Regulation 3.1 b) "A birth certificate for each player, issued by the Registrar-General, indicating the Province in which the player was born and the date on which the player was born, shall be filed by each Centre with the Regional Director or his designate." Please complete this form to submit proof of age and residence the first time you register your player with the Markham Waxers.
